I want to create a form for our Annual Performance Review process for about 3000 people. We have many managers (several hundred) and each manager needs to have their staff do a self evaluation prior to the manager doing his evaluation. Each employee should only be able see their form and each manager needs to be able to see the forms of their employees. I have IP 2007 and MOSS, but not Form Services. I can imagine several ways to do this, but none of them seem very easy to implement, i,e. a folder for each employee with custom permissions will be unweildy. Any suggestions on how to meet these requirements would be greatly appreciated.

I dont know if this is the best way to do it, but what you can do is go into the library where the form will be saved and make "My Documents" as the default value, and then delete all the other views that are in the library, ie delete "All Documents" and "Explorer View".

Then create a view for the managers to use based on groups.

You can use user roles to achieve this one. Add three user roles like

1.       Default- Set this as default one.

2.       Employee- Map this one to Employee name field in form.

3.       Manager- Map this one to Manager Name field in form.

Validate the user while opening the form and open the form only for users having ‘Employee’ or ‘Manager’ roles.
