Submitting Web Form with Signature issue

When submitting a form Infopath does not change the view and does not send the signature for approval view to the end user.  Where the signature suppose to appears, it is blank.  The same thing happens on forms if they have a button.  The button is not visible to the end user once you submit the form.  What we are trying to do is have an end user fill in his/her information and then submit the form to their supervisor for approval.  The supervisor then either rejects the form or approves it.  If the supervisor approved the form, he/she then submits the form to be processed with his/her electronic signature using their Common Access Card.  We have tried everything without success.  When we execute the form locally, it works; however when we publish the form (Web enabled) the signature block disappears.  We tried to send just a link to the library and that does not work.  Has anyone had success with this?  We want to be able to have the person at the end of this process to see the signature of the supervisor.

We are using MOSS and InfoPath 2007

    TIA

    Mike
