How would I make an Action Log?

    Hello,

I have a browser compatible form template that will be used to track items through a Chain of Custody.  I have it working up to this point like so:

There are currently 4 views, when a user logs onto a SharePoint site it opens the form in a page viewer webpart (so it looks like it is built into the page) on View 1.  The view has a Barcode textbox field and a submit button with rules to tell whether to goto view 2 (create a new item to track) or view 3 (update an existing item) depending on if the barcode number has already been submited.  View 2 is essentially where the user enters the initial data and submits it to the Form Library, after submission the form goes back to view 1.  View 3 pulls all the data in via repeating table and filters it so the user is only presented with only the information tied to the barcode number.  All works nicely.

Here's where I am having problems:

When a user gets to view 3 and is viewing the initial data entered I would like to have a simple action log.  I have been playing around with several options, but not with much success.  To explain further, a user will change a dropdown to the appropriate action (received, stored, created, sent out, etc.) and click a button.  That action would then be posted into an "action log"  where it could keep track of all actions performed on the item attached to the barcode.  Get it?  I'm thinking about using a repeating table and a dropdown that will set the a fields value then create a new row... but this requires coding and I'm not sure that is the best option before I go down that road.     To visualize my idea please see the following images (example using PHP and SQL):

    View 1  View 2  View 3

Thanks for any advice, help, how-to's,
