Where I should write this javascript code ??? And I don`t see any links with Sharepoint params... I need to pass to the form ID of document library from wich this form was created...

You can add “Add OnMergeRequest/OnAfterImportCode” in Tools -> Programming ->MicrosoftScriptEditor.
