Data skewed when users fill out forms simultaneously

Hi everyone, I am having an issue in InfoPath 2010 browser based forms, whereby if multiple users try to submit forms simultaneously, the submissions are skewed. Eg. If John Doe fills out a form and Jane Smith fills out a form, and they simultaneously try to submit, John Doe may get an error, and Jane Smith may get a successful submission, but the form will show in the library as Created by Jane Smith, but the contents will be from John Doe's submission, so the results are skewed. Further to this, the workflow email that is sent automatically, will have the right data. What can be causing this. Just to let you know - I created an icon on the site called Add New Issue, and it hyperlinks to the default New Form (Add New Document) as if you clicked that link from the forms library. I am not sure if this is contributing to the issue, but any urgent feedback appreciated, as the solution is very dynamic, and there is a high probability that multiple users can try to submit at the same time.

How are you naming the forms when they submit? If you use something unique for the form name (like a field that you can set one time to the functions now() and userName()) does that solve the issue? It sounds like you may have one user overwriting the other.
