    Creating a customer satisfaction survey that includes The Employees Names and Pictures that worked on the job

     Hello all, I am new to InfoPath and am facing a daunting project. I need to create an emailable survey, that will include some basic questions, and specific questions regarding the employees that worked the job. I need to include their name and picture. The data comes from an Access app, as well as a sharepoint list that contains the employees name and picture.

    Example:

    Please rate each mover on a 1 to 5 scale on their

    MoverName

    MoverPictire

     Appearance: 1,2,3,4,5

    Language: 1,2,3,4,5

    Efficiency: 1,2,3,4,5

    Communications: 1,2,3,4,5

    Attitude: 1,2,3,4,5

    I have managed to crudely get employees names and pictures to show up in the InfoPath form, but they are just there, not related to the Job in any way. Not sure how to do this.

    There will  be several emails sent to different customers on the day of the job. I need to retrieve their email addresses from the database and associate them with the appropriate survey.

    I need some general direction here, I can usually figure things out if pointed in the right direction.

    I thank you in advance.

    Hi topdes1k23,

    I believe that you will need to use a global unique identifier (GUID) in order to associate your rows/data from one another. There is a previous thread which discusses generating GUID using C# or VB, or you can have InfoPath generate a GUID by using a formula similar to this:

    concat(userName(), translate(now(), "T:-", ""))

    So every time you need to pull data, you will only need to pull out the GUID. Hope this helps.
