Help creating a form to put onto sharepoint for intranet site.

So I am fairly new to sharepoint and brand new to infopath and I need some help.

To make this easily understandable, I broke down what I need:

    •    The department each rep did tasks for*
    •    The tasks for each department*
    •    The number of touches for each task*
    •    The number of hours doing the touches for each task
    •    Total amount of hours doing all touches for that day
    •    A comments section for each rep, for possible other tasks, IT outages, etc.
    •    Workflow maybe?

Touches is an internal term for us, which basically means number of items completed for that task

*These items are unique and what have been giving me trouble; reason being that it is very common and most likely that a rep will do touches for different tasks, and different departments all in one day. I am having trouble because with a drop down, you can only choose one, and then only one task, which then would require the form to be submitted multiple times. However, with check boxes, there is no way that I have discovered, to specify what you are specifically doing. For example, when I check mark two departments and then four tasks, how do I discern which touches go with which task with which department? We CANNOT combine the number of touches into a big total.

So that is basically what I have been trying to figure out. And I know there is a way, but I just can’t seem to figure it out. Also, I’m not really sure what it would look like on the back end, so that when each user submits the form, it will show that information accurately.
