Hilary:
Yes, that's the location I want to submit to.
The name of my data connection is called SharePoint Library Submit, which was the default.
My exact Document Library URL is https://sharepoint.sandia.gov/sites/ITSM/Tool/Testing/Forms/AllItems.aspx?RootFolder=%2Fsites%2FITSM%2FTool%2FTesting%2FFoundation&FolderCTID=0x012000A8FFC4ECC87B9045BC74417F4BBB8777&View=%7b4D423FBB-0D10-4327-95C2-3FB8B15D0D8B%7d/ and the default file name is concat("Test Plan for ", txtCustName, " - ", txtSGpName, " - ", rtxtReqSummary), which comes from fields in my form. Allow Overwrite if File Exists is turned off, and the option to make this the default submit connection is also turned off.
I notice that if I set the submit options to Rules, if I save and republish the form, it changes from Perform Custom Action Using Rules to Send Form Data to a Single Destination with SharePoint Document Library being specified, and the data connection listed as SharePoint Library Submit. Since it's the only place I'm submitting to, I guess that's okay.
Should I change the Document Library URL to the standard one, like you typed, instead of the weird SharePoint one?

An imprecisely defined requirement has an infinite number of possible solutions. Unfortunately, only about a maxiumum of 3 solutions are actually satisfactory to the end user.
