Help? Error message on preview: "The form definition (.xsf) file contains an error in the xsf:view element for the following view: Provider View Catastrophic Failure"

When I hit preview, I get:

     Infopath cannot open the form.

The form template is not valid.

    Under more details:

The form definition (.xsf) file contains an error in the xsf:view element for the following view: Provider View
    Catastrophic failure

Any help is greatly appreciated. I cannot seem to find anything about this anywhere.

Ooooh! Catastrophic failure. That isn't helpful at all. Can you try double clicking the .xsn file so the form tries to open and see if you get a different error with additional information?

Here is a screenshot of that.

Thank you for taking a look. Please let me know what other information you may need.

Euw, catastrophic failure again. Want to attach your template under the Options tab? I'll see if I can get it to behave.

It has something to do with the two buttons at the top of the Provider View - when I removed those, it previewed. I'm not sure what the issue is - I didn't see anything right off, but it does seem that one of the buttons has the same control ID as a button on another view (which I have done before in forms w/ code, so I'm not sure why that would cause an issue). At any rate, remove those two buttons (not the Submit one, the other two) and try again - if it previews / opens, add the buttons back on and re-add the logic.

As an aside, you'll want to rename your data source fields so they have meaningful names, and you have the same field for answers to question 16 & 18 in the Provider view - not sure if that is intentional or not - forgive the un-asked for advice. :-)

I did exactly that and it worked. Thank you very much for the help.

And thank you for the advice as well, this is the first time I have ever used infopath, and I think I will continue to use it for many things in the future. Every little bit counts :)
