How to add hyperlinks to related documents

I have a list with two columns, Title, and Document ID

I have a document library with the standard document library fields, and a calculate field called Document ID.  The name of each document starts with a 5 digit number, so the calculation just grabs those first 5 characters.  Each item in the list can have one or more related documents in the library.

I want to be able to go to the list, click on an item, and have a form opens that shows the related documents from the document library.  I want to be able to open the related documents from the form.

I figured if I created a new calculation column in the document library that concatenated the static URL to the library, http://www.sitename.com/subsite/libraryname/, and the document name, documentname.docx, into a single URL = http://www.sitename.com/subsite/libraryname/documentname.docx, and placed that on the form, I would be able to click the hyperlink and it would open the document.  But it doesn't.  I have tried various different calculations, including trying to call the internal document library document ID, but nothing works.  Sometimes Word will open and give me a corrupt contents error, or just a blank page.  I know the documents are not corrupt because I can click on them in the library and they open just fine.

Has anyone been able to accomplish something like this?  It seems like it should be pretty straightforward.
