I have a form that was injected with qRules - then somebody else added code. Do the instructions in the document "Using qRules with Custom Code" apply in this situation? What can I do to restore my qRules cascading dropdowns functionality???

Yes, I think that tutorial should do the trick. You may be able to skip a few steps because the qRules data source might still be there.
