Leave Form, require a user to answer a question

    Hello all,

I'm trying to do the following, when a user selects "Sick Leave" from the type of leaves, a question will pop up and require the user to provide an answer:

(if the person applies for Sick Leave, it has to ask a question ‘Medical Report available?’ and display message ‘Medical Certificate should be submitted any SL over half-a-day’ – when the Person replies YES, then the system has to say, ‘Please submit the Medical Certificate to HR today; This will leave will NOT be considered as Sick Leave unless the Certificate is submitted’

How difficult would that be? Thanks again in advance for everyone's support

Does it need to "pop up"? The simplest way to do what you describe is to have a section, which is hidden by default. This sections holds the additional questions the user will need to answer if Sick Leave is selected. Then when Sick Leave is selected, the conditional formatting displays the section, and thus the additional questions.

Actually I got this all figured out, thanks so much for the support.  I've still got the headache of trying to figure out how to auto-create folders of submitters.  I'm trying to figure out how, when a user from a certain department submits a request, and if the department folder doesn't exist and the user folder doesn't exist, that somehow these two folders get created.  Argh
