What is the question? I think you left off all the rest of the information you meant to post.
I can tell that the concatenation formula above is attempting to construct a Forms Services URL, but it's not a complete URL. It has most of the main components and may work, but it should have more in it. The 3 parameters you listed at the bottom are pieces of metadata from the form that will be pieced together to form the full URL, but I don't know what you're trying to ask us.
