Show / Hide Form Parts for a Newbie

We are trying to design a Technical Support Request form and would like to have some text entry fields appear or disappear based on setting they have previously entered.

For the life of me I can't figure out how to do this.  If you have instructions or an example to share then we would be in your debt.

     Regards,

Hey there -- group controls and labels you want to hide together in sections. Your sections can be bound to groups already in your data source. Sample attached. Right click the link in the header and save the file locally. Right click the saved file and select Design to open in design mode, where you can preview the form. You'll want to take a look at the conditional formatting on the sections.
