(clarified) How do I create list items that fill in fields with a lookup from another list

I have a sharepoint site containing a bunch of lists, List A which contains servers, and List B which contains locations

    eg:
    List A
    Computer 1, Computer 2, Computer 3 etc
    List B
    Site 1, Site 2, Site 3 etc

I am trying to insert items from a workflow (sp2007) into another list (list C) that uses lookups from list A and B

If I were to be filling out the form for adding an item to List C manually, I could pick the items from dropdown boxes,

My form does a lookup on List A, and allows the user to select a region, and passes that field to the workflow via columns. the workflow is expected to create a new item in list C with the value that the user selected. but I cant figure out how to use the 'create list item' workflow action to fill in this field in list C accurately, seems to always come out blank.

resolved this, I had everything correct except the second to last field, it should of been 'title' not 'name (for use in forms)' as 'title' is the column/field in the list I am actually looking at.
