Is there any reason you don't want to use a radio button?

If you use radio buttons, you can use count(evaluation[. = 'O']), count(evaluation[. = 'C']), etc. to aggregate the summary values at the bottom.

The formula would go in the summary field.  I'm assuming you're currently using the sum() function to count the number of checked boxes, so it would be similar to that.  Just use:

     count(/full/path/to/your/radioButton/field[. = 'valueToCount'])

That's the most complete I can make it without actually having your form.

This formula should do the trick:

    count((field764 | field765 | field766)[. = 1])

But I think structuring your form with hundreds of fields named fieldNNNN is not the way to go.  I'll try to send you an example of a better way to do it in the next day or two.

On second thought, the number of questions in your form seems to be few enough that you could get by without using a more structured data source, but I would suggest using more descriptive names for your fields.  It'll help when you're setting up your formulas.
