Your screenshots don't display for me. Can you maybe try attaching those under the Options tab in a reply? Or attach your form?

Yes. You have 4 email submit data connections. In three of them, I can see the text field1 was entered in the subject instead of using the formula editor. I'd go through each of the data connections again and correct this.
