Beginning on page 29 of the qRules User Guide, you'll find additional information for the appropriate data structure of your form for using the SaveToSharePoint or the SaveToDbxl commands. I'm attaching your form with the attributes added. Also, I notice that your test template is marked as using browser compatible features only -- page 23 mentions that this command is unfortunately not currently browser form compatible.
If browser compatibility is a requirement for you, I'm afraid qRules won't work for your scenario.
Also, please note that you can drag the "Error" field from the "QdabraRules" data source onto your form to view any errors that have occurred when running the command.
