Here is an example of a individual document catalog. Use "View Docs" in the Catalog view to view this. This will list all of the documents with links to them. It also has a link to open a new document. It uses paging to improve performance when their are many documents.

Creating a New Document Type in DAT. Name the Document Type and use the Attachment Control to upload your form template. Then Save the configuration and return to the Catalog view.
