Hi - what have you tried thus far? When you say the "three rows below" do you mean model, serial number and delivery status should get set automatically? Won't all serial numbers be unique?

I went and looked around on this site to see if I could find any relevant posts and one mentioned that the template has to be used on a different site, not the same one? Not sure if that is relevant in your case?

Oh dear - I'm sorry, in the past when I've created a template from a list with a customized form, it included the form. It has been a long time since I've done that tho - SharePoint 2013.

I think the only way to do this would be to save the list as a template and then create your new list form the template. In terms of SharePoint Designer workflows I'm not sure if there is an easy way to migrate those or if they just have to be rebuilt.

Is this a list or library form? If a list form, I think the only option would be to save the existing list as a template and then create a new list off if it. If a library form, you could just republish to a new library. In terms of the 404 error, you may want to take a look on the SharePoint Server for that?

Ick. I've seen errors like this before in designer, but haven't used a filler form in a long time. I tried to set up a test, but only have SharePoint online, which won't let me set to IE mode. You aren't using MFA on SharePoint 2013? I actually don't even know if that's an option.... I'm sure you've tried this, but ...
