    Hello and welcome! For problem 1, in order to submit to Access, you need to have based the form off the database when you initially designed it. If you did that, and the controls on the form are bound to the fields in the datafields group in the main data source, you should be able to submit to your table, which will add new records.     I&#39;m afraid I&#39;m at a loss, too. The only thing I can think of is that your Submit connection is not set up to the same site as you are looking at. Perhaps you should copy the url of the document library and walk through setting up the Submit connection again, pasting the url in for your SharePoint url?

    Verify that you are looking at the same library you set the form to submit to. Also, if you haven&#39;t already, refresh the page. You can also look at your Document Library in Explorer view, in case your view is set up with a filter that is preventing you from seeing your submitted form.

    One way to troubleshoot this would be to open the form in design mode and set a field in the form to have the value of the formula. Then, preview the form and see if the formula generates the name you want.
