My only hunch is that the form was injected with the Admin deploy option then sandboxed, or vice-versa (injected with the sandbox option, then admin-deployed).

Ah, yes, the version does make a difference. This feature was added in January of 2013. It looks like your version is from May 17 2011.

1. Is DBXL installed on the SharePoint - 80 site (or on the site where the form library is located)?
2. Does the DBXL DocType use a SharePoint mapping? If not, is the scenario Side-By-Side? (or maybe something else)
3. Could you look up the correlation ID in the ...

For reference in case any one else stumbles across this issue - I believe this was resolved, and the solution was to reinject the forms using the same version of qRules.

Yes, you can do both of these things.
To keep the original attachment, use the parameter /clear=no
To name a file, you can use the parameters /name (non repeating) or /namexpath (repeating attachment).
In your case you'll have to add form name to each row of the schema, so you could use
SaveToSharePoint /url= ...

Office 365 updates have, in the past, caused sandboxed solutions to stop working, but Microsoft eventually addressed those issues. This thread is pretty old, so hopefully all those issues have already been ironed out.
