Removing the default value from the supervisor field fixed the issue. Thank you again for all of your help. I really appreciate it!!!

In the Supervisor field the value is AD ID but the rule is telling it to change the value to User Name. I attached a screen shot of the list in SharePoint that the values are pulling from.

I have the form you attached open. I am looking at the fields associated with Supervisor. In my Rule I am telling it to set the fields value to User Name but the value associated to it in the Drop-down list properties is AD ID. They are different values. The difference is the supreme-pa\ which is the domain name in Active Directory. Am I getting ...

Thank you for your response!! When I change the data source to a value of @User_Name and the Display name to @User_Name it fixes the duplicating problem but it breaks the conditional formatting on the SupervisorApprovalSection.

That is so nice of you. Thank you. I attached the form. To answer your question, yes when the user selects the name of the Supervisor - it is a drop down using the same data source as the list with the user names.

OK. So I am going through the steps. I am at the Specify Filter Conditions window.
When the user is filling out the form they will select the name of the Supervisor.
Not sure I understand what you mean by 'I'm assuming there is some other data in the form that matches something in your list to let you know which user name ...
