I've a form and I have to record a date multiple times.
I've a submit button with a rule to set field Date1. Easy enough.

I don't use the email on submit option.
I tend to submit to a library. If you did that you could have a hyperlink control which could create the email for you. Then you'd see the email before it was sent.

Check the size and width of the controls. Text Boxes make sure they are multi line also. If you're printing a web based form you are very much at the mercy of your browser too.
