Thanks for the info. However the list is monitored often so I have added in a manual start option so that "failed" workflows can be triggered. This will have to do until I can figure out a perm fix.

I have narrowed it down to Office 2007 users. Basically the XP (or office 2003) user can submit their form and the workflow kicks off just fine. When the office 2007 user tries to kick it off it is automatically set to complete. I am wondering if it has any thing to do with the use of InfoPath 2003, I wouldnt have thought it would ...

Yes I have found this issue. It seems to have no pattern that I can find also. I also have users on both XP and Win7. however the workflow will set to complete with XP users too.. this is really frustrating as the workflow works well in most cases but fails around 10% of the time
