    When designing a new form select SharePoint List as the template.  It will prompt you for a Site Location and then a name for the list.       If you are trying to change the created date column you will not be able to do this with InfoPath.  You will need to write a script to run on the List to change those properties.  I prefer PowerShell but others will say Visual Studio.  Whatever makes you more comfortable.

      I think your problem is in the Refunds_Team, you should use the Insert Field or Group, then select your datasource, then open dataFields and select d:SharePointListItem_RW then on concat should be d:Email_Address I would envision it looking like eval(eval(SharePointListItem_RW, 'concat(d:Email_Address,";")'), ...
