    When you change the filename outside of the form, it doesn't change the internal field that controls the filename. If you change the filename in Sharepoint, and then open and resave the form, it uses its internal field as filename and creates a duplicate of the original filename. Is this what is happening?

    What field is being set by the button? A true/false field can be used to check if the form was submitted.  True - submitted, False - not submitted.

    There are ways to make this work, but they take some forethought. As long as you are not deleting fields from the template, the old forms will still function.
