I've published a form to a library and it's been used heavily.  However, we've started seeing forms listed that have lost their employeeid or employeename.  Any idea what can cause this?  Does anyone know of a "fix"?   Thanks Sherry

I've tried using this code in developing a form with an autonumbering field. I created an SQL DB table, with one field (FormID). I'm getting and error when I try to preview and test this code.
