what exactly is your need for the user not to be notified

I have tried both. The alert settings can be reviewed go to site settings and click on <i>My alerts on this site</i> if your alerts are listed correctly then the issue may be with the sharepoint server and its email settings.

It will say form1 when you begin a new doument but use the SAVE AS option off od the FILE menu and name it what ever you want.

is the confirmation box from infopath or outlook?
