 What method did you use to populate the user information?

 I would create a workflow text variable and set it's value using the Build String function in Nintex.  Then use that variable in your email.

 I assumed that the user already had access to the form.  I can't help you with that part. That's a question for a different forum,

 Do you use Office 365 or SharePoint, or do you have some platform already in the cloud for you android device to access?

 When you publish the form are you making the field available in the SharePoint Library?

I believe it is because the submit button is creating a whole new record in the sharepoint library so you end up with the old form (with the old name) and a new form with the new name. I think what you really want is a Save button.
