I tried to break it again by unchecking the all the options under Customize commands once again, but it did not break after I re-published it, so I'm not sure why check an option there fixed it and why unchecking the option won't break it again.

I was able to fix this by checking at least one of the commands in the customize commands window of the repeating table properties on the data tab.
