How is your rule setup to populate the approver name?  You should be able to set an action rule for the attachment field that will populate the approval field.    

You have one too many parentheses at the end of your formula.  Try:   Field 1 - ((Field 1/Field 2) * Field 3)

Same problem here.  Published to a network location, not Sharepoint.  No custom code, security set to Domain, network location is open to all users.  I publish the template and it works for some users.  I re-publish the template and it will work for different users.
