The form does actually submit and a workflow runs sending notifications that the form has been updated.  The modified, and modified by fields are even updated correctly. So its none of the problems you've suggested.  It's a random error that is not possible to reproduce with certainty.

Infopath 2007 has the Logic Inspector...  Tools --> Logic Inspector...  Not sure if 2003 has it as well??
