I'm trying to something similiar, but have not been able to find the answer. 

Where will the code be added?  Will this be a rule in the submit button?  Do you have a sample that you can share?

I have a secondary data source that retreives the 5 data fields.  When I submit the form to the document library the data disappers and it's not saved. 

I have a connection to a secondary data source and  I need to be able to save the data that was retreived from the datasource.   I have the output going to the Sharepoint Document Library.
