If you look on MSDN in the developer reference you should find what you need.

Ok so yes you would need a seperate template for each template version.
You might look at the tool we have on the Qdabra Web Site for migrating botht the template and the forms to a different library.

I like Patrick's Idea.
You could make a set of 50 or whatever your maximum was then just show the user the subset that they need. That may be the easiest way of letting them pre pick a number of sets.

There are a variety of methods.
Low tech would be create a list of Mondays. Then use a filter to display the first one less than today or whatever your target date was.
A more elegant solution would be to you some VBscript or code. I am pretty sure there is a date function to get day of week, so with that you could calculate Mondays.

A backend database would allow you to collect the results in a table for reporting and analysis via web report or Excel.
I am guessing you are creating a group for each question and ...

Ok so you are saying you want an Audit Notes field a Discussed with for each area of compliance.
But you want to have it built for you...
Are you going to save this data into a database or just keep it in the individual form?
Where is the layout text coming from? Are you pasting from Word or something?
I do not think there are a lot of ways ...
