You have multiple actions on a single rule.
I am suggesting spliting some of those actions out onto new rules.
See the New button in your screenshot? Click it and select Action.
Your new action rule can have a condition, like "VoucherNumber is blank" - and then can run your query and set your voucher number field.
Your other rule ...

Yeah, that is why I am saying do the following:
Add a field to the main data source called something like "FileName"
In your submit connection, use that field for your form name, and select the "overwrite existing" checkbox
In your submit rules, where you have the rule to query for your data for the ID and set the ...

Hey there -
You are submitting to SharePoint Library? When you set up your submit connection, you have the option to overwrite existing. If you ensure your file name for the submit will be unique, you can safely do this - take the whole saving thing off the table - users can just submit, and their updates update the existing item.

Here's your form back - I've added to the error handling in the SubmitToSharePointList command. Save the file locally, and then right click on the saved file and select Design to open in design mode. From there you can preview - hopefully you'll find a lot more info in the error node this time - reply here with the error text....

So, your supervisor drop down uses the AD_ID for the value - that means the supervisor node will have the AD_ID for it's value - even though the drop down displays the User_Name.
The filter then, needs to select the User_Name where the AD_ID in the same secondary data source is equal to the supervisor field in the main data source.

Okay, attaching the template to this reply, and more info to the next. To use the template, you need to save it locally. Then you can right click the saved file, and select Design - this will open the form in design mode.
From there, you can save the file locally, preview, etc.

So.... you attached the XML that the form creates, rather than the form template. Can you attach the form template - the XSN file? :-)

Well, you have a SharePoint list - it has user names, among other things. When the user selects the name of the Supervisor - is that drop down using the same data source as the list with the user names?
Do you mind attaching your form to the Options tab? I can probably put this in place for you pretty easily, and then you'll see what I ...

Yup, you just need a filter.
So, on your rule, your value is currently User_Name. Click the fx box to open the formula editor again. Highlight the User_Name field and select Insert Field or Group. IP should open up a data source window with the field selected. In the lower left corner, you'll see a filter data button. Click that.
Now ...
