If your form is opening in InfoPath and not in the browser, the URL is usually:
http://server/site/library/Forms/template.xsn
With the URL to your library up to the /Forms.

Visual Studio 2005 can be used with InfoPath 2003 or 2007 projects for adding managed code - I believe VS 2008 can also be used.
If you have InfoPath 2007 installed, you can use VSTA for managed code development - I think even with the IP 2003 OM (sorry, it has been awhile - I've been using IP 2010 for a while ...

As to your first issue - I'd put the blame on the rules themselves- I've not found rules or conditions to behave inconsistently. I have found them poorly implemented, however. So, I'd make sure your logic to create an ID is sound - and make sure it only runs when there is no ID present - so that existing XML don't end up getting ...

Where is the expression box or field where you are are using this expression?
I'd think the expression would be something like this:
count(../my:Repeating_Row_Overhead[my:Invoice_Receipt = "yes"])

Just so you know - it is possible to preview existing XML with your form. So if you have a form in production, and you are making changes to the template, if you preveiw with one or more of your existing XML, you can quickly ascertain the possible repercussions. Here's a little blog post on this.

Are you selecting the field using the field picker? Is your Office install up-to-date (patched, service packs installed)?
