Effectively, at least as far as I understand it, when a user opens a form for the first time it caches the form locally and references the template. If your form is called the same thing it will not be able to distinguish between the old form library and the new one. I thnik best practice would be to Change the name at the same ...

Right your problem is that you are only selecting the first field in the repeating table, you need to select "all occurances" that should do it.
