Oh - I'm wondering if the two forms were injected with different versions of the DLL? Can you try re-injecting both of them with the version you currently have installed to make sure they both have the same dll?

Okay - so just so I can be sure I understand your precise setup, so I can try to reproduce, your form has a SharePoint mapping, and you are having the issue of failed submits in both filler and browser? Failed submits are only occurring if the form is open, submitted, left open and resubmitted? Does this describe the base issue correctly?

Yes, I think this is possible - I'm guessing your email addresses are currently set as default values?
I think what I'd do is add an attribute to each email field called "Changed" and set it to "true" when the email field is changed. Then for my email "To" field in my submit connection, I'd use a formula ...

Just to add a bit to Ernesto's answer, you can also change the value using a rule - there are some new functions in InfoPath 2010 that include one that will return the site the form is opened from, I believe. You'd also need to add a rule to change the GUID in the ListCollection node, however.

In your data source task pane, find the top most folder. Right click it and you can add a field - this field will be in the main data source, not connected to the database. Make the field the right data type (base 64) and then you should be able to drag and drop it onto your for for an attachment.

Hmmm. I hadn't realized till your last 2 updates that you were talking about a browser form. Do you have the same results in Filler?
Have you tried changing the postback settings on your submit button? Under the button properties, under Browser, set postback to always?

Now I'm being nit-picky and spamming you - such fun!
I tried this exact form in a 2.6 install in our environment, and after each submit (no matter which repeating section I filled out) my changes were saved - I could refresh the documents list in the Documents tab in DAT and see the version incrementing - when I closed and re-opened after ...
-
Also, sorry, not to sound nit-picky - but are you refreshing your view of the documents tab between saves?
-
Are there any errors in the Qdabra Event Log with your additional submits?

It was the XPath you'd used - it was absolute instead of relative:
/dfs:myFields/dfs:dataFields/my:SharePointListItem_RW[1]/my:Project
The above will always get the first item. From the field you were on (Project)
../preceding-sibling::my:SharePointListItem_RW[1]/my:Project
will get you the one above.
I've attached your form with this ...
