After moving and renaming your fields - aren't you experiencing data loss when you re-open the XML with the new template?

If you are using default values, you may want to deselect the option to refresh the default if the values involved change (in the field properties). If this doesn't give the result you want, you may want to use rules instead of default values.

This isn't reproducing for me - see attached. However, your screenshot looks kind of odd compared to mine, doesn't it? See how in yours, Insert Item is over to the right? Is your SharePoint server all up-to-date?

What version of SharePoint are you using? 2007 or 2010?

If you start with a new, blank InfoPath template, you won't be able to submit to a SharePoint List out of the box, so you know. You can submit to a form library, which may be fine for what you need - it really just depends on your requirements.

This is very odd - when I try to download this, I get an XML file instead of an XSN. Perhaps just attach the CAB you got the manifest from?
