You can more precisely control the close event by defining a Rule on a Button. Add the Action "Close the Form" and tick the Checkbox saying "If changes have not been saved, prompt the user to save".This will ask the user if any changes done in the form.

Hi -- There is also a sample here that may help you. This sample form copies the attachment to a SharePoint document library and creates a link in your form to the attachment. Give it a try -- let me know if you need help with it!

I think I see the confusion now -- I'd intended for you to reply to my email with the file. You can also place it in the community uploads section of the Downloads on this site.

I think you have added only one user role and the form will be defaulted to that role for all users. In this case the condition you have put on that field may always fail or always suceeded. To solve this problem add one more user role and default the form to that user role.
