The issue with your mapping is that you are not using relative paths.
Where you have the XPaths that start with my:myFields, you need a relative path instead.
../../my:NonRep/my:CityStateZip
for example. You have to manually change those - you can't use the field picker in the task pane.

Under the options tab you can attach a file. Can you attach a screen shot of the mapping tool with your mapping? Or the mapping.xml file?

You bet - it can be done, but you have to hand enter the relative XPath to the field in the mapping tool when you create the mapping. So, if your data structure was:
my:myFields
my:CompanyName
my:group

Jinx - we figured it out at the same time - again, I had it wrong in my reply too, didn't even notice it till I compared the XPath side by side.

Ah ha! I had the namespace wrong in my first reply - we want "my" not ns1. :)

I changed the default value for the Question field in the main data source:
xdXDocument:GetDOM("TTRUpdate")/ns1:TTR_Update_Questions/ns1:Question[count(current()/../preceding-sibling::my:Questions) + 1]/ns1:Point
As I mention above, you'd ...

You are getting an error with the copy of the template I attached with my reply?

Okay. Here is what I believe the heart of your problem is - you are trying to update an existing item with providing qRules that item's ID. In a SharePoint list, each item has a unique ID. qRules offers a way to populate the XML with a newly created item's ID number. This allows qRules to update the item.
In your form, I think you are ...

It was a simple typo - you were using this: current()/../preceding-sibling::ns1:Question
It needed to be this: current()/../preceding-sibling::ns1:Questions
Questions is your repeating group.

I don't see your form attached? Can you attach it, or a screenshot of your main data source?
