You can remove the toolbar altogether and just use buttons - one for closing the form and the other for submitting the form. Go to Tools --> Form Options --> Browser (should be the first one you see) and uncheck the boxes next to Show Toolbar on top and bottom of form. Hi -- you want the site name the form is published to?
The formula:
substring-after(/processing-instruction()[local-name(.) = "mso-infoPathSolution"], 'href="')
will get you the url of the published template. You can further modify it to get just the site using other string functions....

Hi -- you should be able to add fields to the data source for a form that was created from a database -- but only to the root node. If you select the top folder in your form's main data source, you should have the option to add a field.
