    So I've followed the steps outlined above (and recreated many times) but I have been unsuccessful in getting this to work.  The form says it submits correctly but nothing is ever added to the list.  Any ideas why this is happening or how to troubleshoot?

    I see that this would save in C drive but what if you have the form on Sharepoint and you want to change the filename when the user saves the file?  Will the coding be the same as above or would there be some changes?
