All of your code is still in your button click event? You originally had this attached to a 'refresh' button, right?

I added the space to in front of output, and added the {d \...} and then the form wouldn't open. I looked over everything and found one missing quotation mark that I added but didn't help. Maybe you can see something that I may have overlooked.

You need to properly filter based on the CustomerID in you form, and those selections must exactly match what is in the SharePoint list so that the filter works.

Do you have a Tools->Data Connection Retrieve data SharePoint List set up?

InfoPath 2007 has this option more clearly in the wizard, so you must be using InfoPath 2003.
My recollection is tha tthis is a "piece of cake" with tricky frosting! Just delete the form name "frosting" in the wizard and you won't get an attachment. :)
