Can you temporarily put a calculated value on your form to make sure this formula is returning a correct & valid URL?

The thing is that InfoPath is only going to look at the first instance of CCSSubApp (for example) as you currently have the formatting rule - but you can change that using the options shown in the attached screenshot. For example, you probably want to hide the button if "Any occurrence of CSSubApp" is blank...

Under the options tab you can attach your template - I may be able to take a quick look.

Well - the file location for the code is under Form Options --> Programming (also from the Developer tab on the Ribbon --> Language). I believe in InfoPath Options you can also change the default location for code files.
When publishing, however, you don't have to have access to these files - you'll notice on the dialog there are ...

I'm attaching a super simple sample. I used default values to add extra instances of my repeating groups - you can use qRules to create a data driven form - try searching for that on our YouTube channel, as there may be some videos out there. You ...

I don't really understand the question, I'm afraid. Is this column in your list? The same one you are submitting to? If so, perhaps instead of your formula you could have a hyperlink column and submit from your form to that (but I guess that doesn't handle for the situation where there isn't a form for the list item - unless you ...

Hmmm - I don't understand what you are saying (where does the form get sent if it is "disconnected" from the DB?) but if it follows the user the obvious culprit is permissions....
