Hi Dean,
It sounds like the ID of the item in SharePoint is what is showing here perhaps? Is it possible that someone is deleting items from the SharePoint list and this could be causing the issue? Just a thought.

I have a document library that saves infopath forms. When a user clicks the link of the form in the document library, the form should be open in the portal. Is this possible ? Can anyone tell me how to do this or suggest me any article for this?
