How odd - the other views don't have the default view set as the print view, do they?
-
This will require code or third party tools.
-
So - versioning is on in the library - doesn't checking the versions show the account making the updates?
-
You'll need another field for this - like a status field, perhaps that lets you know when the form has been submitted? Or perhaps your hidden text field gets set on submit, so it is blank until the first submit? If so - you could use that field in your validation rule - just add another condition that the validation applies only if the hidden ...

Not out of the box, I'm afraid. You'll need to write code for this, or use third party tools.

Feel free to attach your form under the options tab or email it to me and I'll take a look.

You don't have a space between "GetAttachmentAttribute" and "/xpath". You need a space there.
-
Since you are using FormsViewer, you can use the GetAttachmentAttribute command: GetAttachmentAttribute /xpath=/my:myFields/my:FileAttachment /attr=Name will return the attachment name. So I'd just run that in a rule on the attachment field and then store the name in another field.....

Oh good news - glad you got it figured out, and thanks for posting the resolution!

This isn't going to be possible out of the box (if I understand your desired outcome correctly - you want to promote the name of the attachment to the library columns?). You'd need to use third-party tools (like qRules, FormsViewer) or custom code to get the attachment name.
