Your Question and Answer fields could be in a repeating group. Under Data > Defaults, you can add as many instances of the group as you need. I'm attaching a sample. Save it locally, right click and select design - from there you can preview.

qRules cannot be used in SharePoint list forms - in fact, no custom code can be added to SharePoint List forms. If this means you will be unable to use qRules, please contact us through our website to help with a refund.

I'd think maybe:
../preceding-sibling::my:MaterialItems/my:SystemBoundaries

Well - I usually just add a field to the form's data source named Status or some such - it doesn't have to display in the view. It can be blank by default and then in your submit rules, you can set it to "Submitted". Then when the form is reopened, you can switch views if Status = Submitted.
You can bind as many controls as you ...
-
You can change the field the calendar control is bound to so that the data type is just Date.
Or, if for some reason you need the field to be date time:
concat(FORUM, "-", ACTION TYPE, "-", substring-before(IDENTIFIED, "T"))
-
You could have the server admin check the SharePoint logs for the time you were trying to publish, see if there is more info there.
-
You are on the right track - if you want the secondary data to be saved, you have to copy it into your main data source. Here's an MSDN link that may help you with the code:
http://msdn.microsoft.com/en-us/library/office/aa945227.aspx
qRules can also help you with this.

Hi - did Jimmy's answer here: http://www.infopathdev.com/forums/p/27183/95340.aspx#95340 address this?

Thanks so much for the info & update, Mark!

Add validation rules to the fields for the 5 additional pieces that makes them only required if the other field = Immediate Repair. You can also put the fields inside a section that you hide unless the other field = Immediate Repair.
