Ok, I think I understand. It's a question of version. In SharePoint 2010 there's a ribbon. Notice that the Form Options screen says "When the Ribbon is unavailable". Well, the Ribbon is available in ShP 2010, but not in MOSS 2007. So if you publish the form to MOSS 2007, you'll see the toolbar at the bottom of the form. But ...

In the browser, you should be able to add the top and bottom bars.
In the client, only the top bar will show.

Sadly, I don't think there are any such options out of the box for your request, so if you found a blog post with sample custom code, that might be the only way to go. A lot can be accomplished via code, but it's not easy.
How long is your source list? Depending on how many customer numbers there are, perhaps a dropdown would be ...

Is this happening for all forms? What about if you try a new, blank form, and publish it to a new form library, and open in the browser, does it only show the top toolbar?

The easy way to do this is to just have a field in your schema called my:subject. You can set the value of this field via rules, to make sure the value is set as desired. The subject field won't be on the form canvas - only in the schema, so the user won't interact with it.

When doing so, please clarify what you mean by "somebody from authrorized group", that is, how you plan on detecting this (e.g. via an Active Directory web service, via a SharePoint web service, etc.)

You can't add a row to the repeating table on the fly without using code (or qRules). You can, however, define the default values so the repeating table has two rows by default.
